Once you have made your initial appointment, you can click on the links below to download your New Patient Forms. Please print and complete the forms prior to your arrival. This will speed up your registration.

Insurance coverage:
We accept most medical insurance plans. Please call us at 518.450.1080 to verify whether or not we accept a particular insurance company’s coverage. Deductibles, co-pay and coinsurance vary by plan, and it is the patient's responsibility to know the coverage their plan offers. Carriers we accept include the following:
Aetna, Blueshield of Northeastern NY, Bluecross/Blue Shield, CDPHP, Cigna, Empire BCBS, Empire Plan, GHI HMO Select, Health Now, Humana, Medicare, MVP, Today's Options, Tricare, and United Healthcare.
